Cholangiocarcinoma is a rare and aggressive malignancy of the biliary tract that typically metastasizes to the liver, lungs, peritoneum, or lymph nodes. Metastasis to the urinary bladder is exceptionally uncommon, with only a few cases reported in the literature. We present the case of a 70-year-old woman previously treated for extrahepatic cholangiocarcinoma, who presented five years later with painless gross hematuria. Imaging revealed a bladder wall lesion, and transurethral resection confirmed adenocarcinoma consistent with her prior diagnosis. This case highlights the importance of considering metastatic disease in the differential diagnosis of new bladder lesions in patients with a known history of cancer.
