Paratesticular schwannomas are exceptionally rare benign tumors of neural origin that may mimic malignant scrotal masses both clinically and radiologically. Due to their nonspecific presentation, they often present a diagnostic challenge.

We report the case of a 48-year-old male presenting with a painless, slowly enlarging right scrotal mass. Tumor markers were within normal limits, and imaging revealed a well-defined extratesticular lesion suggestive of malignancy. A scrotal approach was used for surgical excision due to clear separation of the mass from the testis. Histopathological evaluation confirmed the diagnosis of benign schwannoma.

Given their rarity and imaging overlap with malignant tumors, paratesticular schwannomas often lead to unnecessary orchiectomy. This case supports a conservative, testis-sparing surgical strategy in cases with benign imaging features.

Paratesticular schwannomas should be considered in the differential diagnosis of painless extratesticular masses. Accurate preoperative assessment and intraoperative judgment can help preserve testicular tissue and avoid overtreatment.
